I saw Ivan Sutherland experimenting with a head mounted display on the TX-2 when I was there (between July 1966 and Sept 1969, most likely in 1968). I suggest you ask Ivan (now at Sun, I believe). He continued that work at the University of Utah, but not (as far as I recall) at E&S.
