I don't know about the second criteria, but there were certainly plenty of people involved in the beta test here on Delphi when the MCI/Newscorp transition to a GUI was still alive.

There were easy access links to provide feedback. However, without having been on the inside, I can't say how the feedback was integrated into updates. I know that some of it was because some identified problems tended to disappear from one version to the next.

It's possible that too much beta testing was done (for the present time-deadline environment). The venture failed, from what I've heard, when the perception was accepted by top management that the necessary launch date could not be achieved and that the product would not be competitively positioned. I'm guessing as a comparative outsider.
